Application Form

1. Download the application form for prices and times - pdf - word doc.
2. Print out the application and fill it out completely.
   
3. Mail the completed application with the deposit to:

4. Emailed forms will not be accepted.

Further Steps

1. You will receive your confirmation letter with the following forms:
      Hospital Release
      Bear Creek Stables Release
   
2. Print out each form and fill it out completely.
3. Both formsmust  be notarized for your child to ride.
4. Mail both completed forms along with any balance and a copy of your child's shot record to the above address no later than 10 days before camp.
5. Your childmay not  ride without all three forms - Bear Creek Release (notarized), Hospital Release (notarized), and shot record.
6. Emailed forms will not be accepted.
